Minnesota Hockey District 5

District 5 Game # and team names must be listed on game score sheets.  Official game # and team names can be found at: http://district5.pucksystems2.com/page/show/14768-league-schedules for each league within District 5.  Note that missing information will result in the host association being fined $10 per occurrence.

Winning coach / team manager must submit the original score sheet via US Mail within 5 days of the date of the game. If a game ends in a tie, the home team shall be responsible for the score sheet. If the sheet is postmarked later than 5 days after game date, the responsible Association will be assessed a $50.00 penalty.
