Minnesota Hockey District 5

Background screening forms are required for ALL association Board Members, Team Managers, and Coaches.  If a person is BOTH a Board Member and a Team Coach / Manager they will need to fill out 2 forms (one for each position). 

Completed forms need to be mailed to Doug Kephart, 71052 CSAH 18, Dassel, MN 55325 as soon as possible.  All coaching forms are due at roster signing day.  Rosters will NOT be signed for those Associations that have not properly submitted their screening forms.

Forms are available from the President of each association.
